STEP 1: Retreat Registration Form
All workshop participants must submit this form to be formally registered and approved for class. Any sensitive information provided will be held in strictest confidence.

STEP 2: Retreat Payment

Once approved for class, all registrants will be emailed payment instructions for the initial non-refundable deposit of US $500.
Payment options include Venmo, Zelle, Money Order, and bank/personal check on US bank.
NOTE: full fee payment must be made no later than May 15th, 2024.

If you are currently under a psychiatrist's or psychologist's care, please note whether you're taking anti-psychotic and anti-anxiety medications. If yes, please describe. (Note: all responses will be held in strict confidence).

Do you have any experience with Spirit Communication (whether passive or active)? If so, please briefly describe, including any classes you may have taken, from whom and approximately how long ago.

What do you hope to take away from this workshop?
