STEP 1: Retreat Registration Form
All workshop participants must submit this form to be formally registered and approved for class. Any sensitive information provided will be held in strictest confidence.

STEP 2: Retreat Payment

Once approved for class, all registrants will be emailed payment instructions for the initial non-refundable deposit of US $500.
Payment options include Venmo, Zelle, Money Order, and bank/personal check on US bank.
NOTE: full fee payment must be made no later than May 15th, 2024.

If you are currently under a psychiatrist’s or psychologist’s care, please note whether you’re taking anti-psychotic and anti-anxiety medications. If yes, please describe. (Note: all responses will be held in strict confidence).
